What is MY PI?
MY PI (PI stands for Performance Improvement) is your online access to the Diabetes PI program. This page will keep track of where you left off so you can go directly back to that step when you return.
What is a master certificate?
The master certificate is awarded for completion of all Performance Improvement (PI) stages in succession.
I am not a primary care doctor and have no control over diabetes management. Do I still qualify for Performance Improvement (PI) CME?
Yes, as long as you see patients that have diabetes and affect how they are managed. For instance, if you discuss exercise with your patients with obesity and manage them with pain medications that would allow them to exercise and improve their care of diabetes you may qualify for this performance improvement initiative.
Patient Data Collection Form
How can I submit my patient data collection forms?
You must be registered for PI in order to submit your patient data collection forms.
How many patient data collection forms do I submit?
You must complete a one-page form for 20 of your current diabetes patients.
Can I use the patient data collection form during patient visits?
Yes. You may find it convenient to print out the form and attach to your patients’ charts to complete in conjunction with a patient visit. You can then use as a reference during data entry of these forms online.
Am I violating HIPAA guidelines by submitting a patient data collection form?
No. In our patient data collection forms we do not ask that you enter the patient name, social security number or anything that would allow us to track or identify a patient therefore you would not be violating the privacy/hippa act.
